Fair and Consistent Health and Care Funding Pathways
This is a timed and targeted piece of work
that will end once the reviews have been undertaken.
Maximum independence is achieved through having the right model of
support, enabling people to be supported with their health and
social care needs, even where complex. This project is directed to
the people who have the highest levels of needs in our health and
care system, and will provide a tool to ensure fair and consistent
way of identifying a person’s health and social care needs.
Through this model we will be able to present the CCG an assured
methodology to reach agreement to appropriately fund and commission
for an individual’s needs:
• High cost / complex care packages
• S117 High cost / complex care packages
