Herefordshire and Worcestershire Sustainability and Transformation Partnership (STP) - statement of commitment to progress the development of an integrated care system (ICS)
To update the Health and Wellbeing Board on
the work to improve health and wellbeing through the Herefordshire
and Worcestershire Sustainability and Transformation Partnership
(STP), in particular, progress towards realising integration of
health and care.
It is a function of the Health and Wellbeing Board to 'review
whether the commissioning plans and arrangements for the NHS,
public health and social care are in line with and have given due
regard to the health and wellbeing strategy'. This update is
intended for the Health and Wellbeing Board to consider the ICS
commitment statement and seek assurance that it will enable the
local health and care system to discharge its duty to deliver the
HWB strategy.

Public engagement activity regarding the STP
plan took place during 2017. Following consideration of the plan in
June 2017, there has been regular reporting to the health and
wellbeing board on integration and transformation of the health and
care system to ensure that the plan and ensuing developments are
aligned with the health and wellbeing strategy, and will support
improved health and wellbeing for the people of
Herefordshire.
Health and care partners are participating in workshops and
meetings alongside members of the health and wellbeing board
collectively as system leaders to lay the foundations for this
work.
